Personal Declaration

The personal declaration must be completed in your own handwriting. Use the correct legal name of each member of your household as it appears on the social security card. All adult members of the household must sign in below certifying the information pertaining to them. Please print clearly.

Please note that you will also need to have an ID card, a social security card, and your birth certificate to complete this application.
